When to move from general care to a spine consultant

Not all back pain needs a consultant — but some cases benefit from one early. A simple framework to know where you stand.

Family physicians and physiotherapists manage most back and neck pain very well. A spine consultant adds value in specific situations — knowing them saves you both unnecessary visits and unnecessary delays.

Reasonable to manage with general care

  • A first or occasional episode of back or neck pain without warning signs
  • Pain clearly linked to strain that is already improving
  • Mild, short-lived symptoms without radiation into a limb

Consider a spine consultation when

  • Pain persists beyond about six weeks despite appropriate care
  • Pain, numbness or weakness radiates into an arm or leg
  • Episodes keep recurring and limiting your life
  • Imaging has been done and you want it interpreted against your actual examination
  • Surgery has been proposed and you want the reasoning examined — or a second opinion

What early specialist input changes

It is not about rushing toward procedures — usually the opposite. A precise diagnosis early prevents months of untargeted treatment, identifies the minority of cases that need timely intervention, and gives the majority a confident, structured conservative plan.
